Requesting Mediation Services

 

To request mediation services, please complete the Mediation Request Form below. State the issues clearly and concisely. 

If your request is related to an adverse determination letter from a USDA agency, the Mediation Request Form must be submitted to the MAMP within 30 calendar days from the date of the letter.

Once you file your form, you will be contacted by a case administrator from one of the above two Community Dispute Resolution Program centers responsible for arranging the mediation, depending on your locale.

Persons with disabilities who require program information in alternative formats (Braille, large print, audio tape, etc.) should contact the USDA's TARGET center at 202/720-2600 (voice and TDD).
